Rate of Pay: $62,400.00- $69,000.00

Responsibilities:
- Provide Clinical Social work services to patients in a Home Health and Hospice care setting
- Assess patients' mental health needs and develop treatment plans
- Collaborate with healthcare professionals to ensure comprehensive care
Requirements:
- Bachelor's degree in Social Work or related field
- Licensure as a Clinical Social Worker
- Weekend availability and flexibility to work Monday to Friday
