Job Description
In this position, you will be responsible for the initial and ongoing bio-psychosocial assessments on the patient and their family. Partner with IDT in establishing and maintaining a Plan of Care for each patient/family. Provide admission assessments social work documentation and make visitation/phone calls as directed by plan of care.
Job Qualifications:
1. Must possess a Master of Social Work Degree from a graduate school of social work accredited by the Council on Social Work Education.
2. Knowledge of group process and ability to facilitate an interdisciplinary approach to hospice and palliative care.
3. Highly skilled in both written and verbal communication.
4. Knowledge of community resources.
5. Knowledge of child and elder abuse reporting requirements and other significant legislation and laws regulating Hospice and Palliative practices.
6. Knowledge and understanding of individual, child and family development, human behavior and communication, the influence of culture and values in Social Work practice. Experience as a medical or hospice Social Worker preferred.
