LMSW Hospice Social Worker

PRN PAY RATES. MILEAGE REIMBURSEMENT.

Responsibilities:
- Conduct assessments and develop treatment plans based on client needs and goals
- Collaborate with interdisciplinary team members to coordinate care and ensure comprehensive treatment
- Assist clients in accessing community resources and support services
- Conduct crisis intervention and provide emotional support to clients in need
- Maintain accurate and up-to-date client records and documentation
- Stay informed about current research and best practices in the field of social work

Requirements:
- Graduate of an accredited institution, MSW or equivalent education/training required in social work and bereavement services
- Valid state licensure as a Social Worker, FULL LICENSE LMSW
- Hospice experience
- Strong communication and interpersonal skills
- Ability to work effectively as part of a team
- Excellent problem-solving and decision-making abilities
- Compassionate and empathetic approach to patient care
- Familiarity with case management principles and practices

Job Types: Part-time, Temporary

Pay: From $35.00 per hour

Expected hours: 2 per week
