TOUR 2 (9am – 5pm)

PURPOSE OF POSITION:

To provide psychosocial assessments and the completion of the HRA2010e for homeless HIV patients.

AREAS OF RESPONSIBILITIES:

All HIV patients that are in need of permanent, safe housing that are within the CHAT Program (Comprehensive Health Assessment Team).

SUMMARY OF DUTIES AND JOB RESPONSIBILITIES:

• Serves as a member of interdisciplinary team to assess housing needs of persons with HIV related illnesses.
• Formulates psychosocial evaluations of clients.
• Prepares HRA 2010E Housing applications and utilizes PACT Program.
• Provides concrete services and referrals as necessary.
• Documents all interventions and activities in the patient’s electronic record in accordance with Departmental policies.
• Maintains statistical data regarding interventions using Microsoft Excel, Microsoft Access and HASA Web.
• Attends necessary meetings in both Social Work and Comprehensive Health Assessment Team.
• Performs other related functions as determined by Supervisor.

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
