Responsibilities:
-To provide supervision to Personal Care Assistants in Home Care setting
-Evaluate and assess the social and emotional factors related to the client’s health problem as a means of estimating the client’s capacity and potential to cope with the problems of daily living.
-Participate in the development and/or revision Plan of Care. Prepare social histories to augment existing service, or as a guide in determining or changing level of service. Counsel, support, mobilize, and work with clients and families to ensure basic human needs, protection and to assess compliance with the plan of care, etc.
Qualifications:
- Bachelor's or Master's degree in Social Work or a related field.
- Valid licensure or certification as a Social Worker (if required by state regulations).
- Experience working in child welfare, workforce development, behavioral health, or related fields is preferred.
- Strong knowledge of social work principles, theories, and practices.
- Excellent communication and interpersonal skills to establish rapport with clients from diverse backgrounds.
- Ability to assess client needs and develop appropriate intervention plans.
- Proficiency in crisis intervention techniques and the ability to remain calm under pressure.
