Job Description
Williamsville Suburban is looking for a Social Worker/Discharge Planner. The primary purpose of this position is to meet the psychosocial and discharge needs of the clients in accordance with current federal, state, and local standards, guidelines and regulations, as well as our established policies and procedures.
Responsibilities for this position include but are not limited to:
•Start and coordinate discharge process when appropriate.
•Meet with clients and families to determine appropriate pathway for the resident and patient.
•Accurately complete and assess the MDS for residents and patients, as well as required chart documentation.
•Serve as Discharge Planner on assigned units. Introduce resources designed to meet the needs of the elderly after discharge and make appropriate referrals and arrangements for services.
•Consult and keep Director informed of activities, needs and problems related to caseload. Work with Facility team to resolve complaints, abuse allegations, and referrals to adult protective services
•Assure that medically related emotional and social needs of residents are met. Provide emotional support to clients and families. Qualifications and Skill Requirements for this position include:
•Possess a Bachelor’s Degree from an accredited school of Social Work. •Minimum of 2 years supervised social work experience in a skilled nursing facility preferred.
•Must have strong written and verbal communication skills, including active listening.
•Must possess the ability to deal tactfully with personnel, clients, family members, visitors, government agencies/personnel, and the general public.
•Must possess leadership skills and ability/willingness to work harmoniously with team members.
