Licensed/Certified Social Worker – Hospital Setting

Description: several positions available in inpatient and outpatient hospital settings as well as mobile crisis units

Requirements:

• LMSW or LCSW- NY state certified
• Experience in a hospital/healthcare setting
• Discharge planning 
• Crisis intervention
• Psychosocial assessments
• Risk assessments
• Knowledge of community agencies and resources

Case Manager

Description: 

Assist Social Workers with discharge planning and follow-up. Conduct and complete intakes, assessments, care coordination/treatment plans. Make home visits to adults to assess their needs.

Qualifications:

• Related Bachelor’s degree in social work, psychology, or related field
• Minimum 1 year of case management experience in a healthcare/mental health setting
• Experience working with SMI population
