Job Summary and QualificationsAs a Social Worker in Case Management at our facility you will be joining a team that has a passion for creating positive patient interactions and a dedication to caring for the multi-faceted wellness of those we serve. In this role you will:
- Be responsible for the day-to-day direction of social service patient care, including quality care of the patient from admission to discharge.
- Evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
- Act as a liaison between the facility and resources external to the organization.
- Implement timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.
- Provide crisis intervention and support.
- Assist the Case Manager in facilitating patient movement across the continuum of care and identify and track barriers to patient throughput.
- Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.
Qualifications:
- Bachelors degree in Social Work required; Masters degree preferred.
- One of the following current State of Utah Social Worker licenses: Social Service Worker (SSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW).
- 2 or more years of clinical experience preferred.
- Knowledge of Utilization Review, DRG review, and Discharge Planning preferred.
