Responsibilities
Supporting residents of RiverMead during times of transition and/or time of need. Assess resident's needs and offer support and solutions that RiverMead or outside providers can offer. Work with Care Planning Team to identify residents in need and support them through change. Work with resident's families, as appropriate.
Qualifications
LCW, LCSW and MSW encouraged to apply.
Experience with elders and dementia a plus, but not required.
