The Opportunity: LICSW - MGB Integrated Care
Why This Role Is Important
This is an exciting opportunity to work on-site, as well as remotely, with a new team that will offer in-person and virtual care to our patients. We are looking for an enthusiastic and passionate LICSW to become part of our Integrated Behavioral Health Service; working closely with Primary Care and other specialty services to deliver exceptional care to our patients across the age spectrum. The LICSW is an integral part of our team, embracing our culture of well-being and kindness, and contributing to constant improvement. Responsibilities include a hybrid of on-site and off-site work with the goal of providing a seamless, integrated experience for patients and care teams. This role will be part of a team that will frequently prototype new ideas, including digital tools, care team roles, and workflows, that may be used in designing for the future of ambulatory care across MGB Integrated Care. What You'll Work On
The LICSW will be primarily accountable for:
Interviewing new patients to aid in the identification of the best treatment options for them.
Supporting patients in accessing resources and managing care
Provide brief intervention to patients awaiting long term treatment
Working closely with the other members from the Behavioral Health Team as well as the Primary Care and Collaborative Care Teams to coordinate services needed to improve the lives of our patients.
Serving as a process advocate and coach in the practice and organization
Improving patient experience by interacting with patients in a professional, friendly manner consistent with their needs
LICSW Responsibilities Include:
Developing comprehensive assessments and treatment plans for our patients (may happen in person or virtually)
Providing a range of interventions such as short-term, evidence-based psychotherapy (individual, families and groups); crisis intervention; care coordination; information; referrals; and safety planning.
Documenting clinical encounter in the patient's medical record in a timely manner.
Participating in formal and informal case reviews, seminars and / or inter- and intra-departmental peer review meetings.
Participating in on-call responsibilities as assigned
The LICSW is also responsible for other roles consistent with his/her training and skill
Knowledge of specific medical and psychiatric illnesses, procedures and treatments, related medications, acuity versus chronicity, effect on normal growth and development specific to service.
Knowledge of age specific behaviors, cultural issues and impact of illness on patients and families.

About You
MSW from an accredited school of social work required
New Hampshire license at LICSW level
Strong assessment and treatment skills
Demonstrated ability to communicate effectively, both orally and in writing
Strong interpersonal skills enabling effective collaboration
Demonstrated ability to understand the role of the Social Worker in a health care setting and to work independently in a complex, fast-paced environment
Proficiency with an EHR; Epic experience preferred
