Job Description
Qualifications:
- Degree in Social Services or related field
- Ability to work as a productive member of a multi-disciplinary team
- Understanding and working knowledge of behaviors, causes and treatment interventions
- Knowledge of local support and community resources
Duties:
- Organize and conduct resident care conferences. Through the conference determine compliance and accuracy of shower preferences, adequate eyewear, the notification of room movies. Ensure residents understand this is the opportunity to participate in the planning of their care.
- Provide in-services on social services related to resident rights.
- Assist to maintain highest level of social and psychological needs of the resident needs: emotional, physical, financial, legal, emotional etc.
- Document interventions in the medical record that meet the resident's psychological needs
- Advocate for resident rights
- Provide for, or arrange grief and/or loss of function support, and other identified problems.
- Coordinate mental health referrals
- Participate in the psychoactive medication committee. Assist to identify non-medical interventions to consider for behaviors.
- Participate in quarterly QAA committee and monthly QAPI
- Discharge planning
- Provide crisis intervention for residents when appropraite and as requested by the multi-disciplinary team.
- Participate, initiate, resolve psychosocial harm monitoring as a result of an incident, behavior, or otherwise.
- Use assessment and care planning to determine and establish plan to meet residents needs
- Through assessment, determine resident's needs for adaptive equipment, clothing and personal items
- Provide resources for residents and families regarding POLST, POA, guardianships, application for Medicaid, End of life, hospice, voting, public transportation and the Ombudsman program.
- Track PASSR's for any necessary changes, forward requests as appropriate, determine accuracy.
- Activate facility abuse, neglect and misappropriation policy and assist with investigation as directed.
