SalaryHCP Talent is collaborating with a Long-Term Care/Rehabilitation (720 beds) in the Bronx, NY that is seeking a Soical Worker (SW) for a full-time Day shift (7a-3p) M-F position.
SW Duties and Responsibilities:
- Responsible for 1 sub-acute unit and 1 long term care unit (80 beds in total)
- Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
- Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources
- Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
- Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
- Liaison between family and other interdisciplinary team members

Qualifications & Requirements:
- Masters in Social Work
- 1+ Year of Long-Term Care Experience is Required
