The License Social Worker reflects the mission, vision, and values of the company, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Shift: Days 8am-5pm
Responsibilities:
- Provide direct clinical services to patients, including assessment, diagnosis, treatment planning, and counseling.
- Conduct psychosocial assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Travel to patients' homes for assessments, follow-up visits, and crisis intervention.
- Adhere to ethical and legal standards in social work practice.
- Participate in case conferences and team meetings.
- Document patient interactions accurately and timely.
- Assist patients in accessing community resources and support services.
- Provide crisis intervention and support during emergencies.
- Engage in ongoing professional development and training.
- Psych therapy

Qualifications
Required:
- BSN
- 3 years experience in relevant role.
- 1 year or direct experience
- Current BLS through the American Heart Association required.
- Licensed Clinical Social Worker (IL).
