River Garden Senior Services is currently seeking a Social Service Administrative Assistant.
The primary purpose of the Transition Care Assistant is to provide support to our Social Services team with required documentation and communications and assist short stay patients and their families with successful transition from our facility to home or other sites within the community. As a Social Service Administrative Assistant
you will be a member of our multi-disciplinary health care team that provides services to our long-term residents and short stay patients.
Qualifications:
- Excellent written and verbal skills
- Proficient with Microsoft Software
- Strong customer service skills
- Working knowledge of locally available community resources
- Ability to communicate effectively with older adults, family members, and agency referral sources.
Education:
High school graduate or equivalent
Work Experience:
At least two years of experience in office or medical setting preferred.
