POSITION SUMMARY: 
The  Skill Discovery-Direct Service Paraprofessional (DSP) provides direct support to program participants including but not limited to ensuring the overall health and safety, teaching skills to help improve our person centered participants to achieving employment and living with a better quality of life, protecting the participant’s rights, implementing individual program plans, supporting the participant in achieving outcomes as outlined in the CSSP addendum, and providing supervision. 

MAJOR AREAS OF RESPONSIBILITY: 
  • Acts as the primary support for implementing participant centered outcomes. 
  • Assists with vocational assessment for all new participants in community-based employment settings including implementing on-site procedures for assessing participant (persons) job skills, providing input for assessments of program participants. 
  • Participate in Expanded Support Team meetings and/or provide information/input to assist in developing individual program plans (IPP) and assist in developing appropriate objectives to achieve stated goals. Follow procedures and strategies to implement objectives. 
  • Assists in developing plans and strategies to address individual participant vulnerabilities. 
  • Assists interdisciplinary team in making appropriate placement decisions in community job sites. 
  • Conducts workplace review and job orientation in order to develop on-site training program requirements. Suggests strategies for job accommodation or modification to better fit the participant’s skills, abilities, and interests, and meets employer job requirements. 
  • Assist in providing domestic, community and leisure skills training and teaching to persons served, consistent with individual CSSP/Addendum, SMA, IAPP goals, objectives and schedules. 
  • Observes and records participant’s progress and behavior as directed. 
  • Provides input into individual CSSP/Addendum, SMA, IAPP goals, objectives and planning as directed. 
  • Assists with physical therapy and occupational therapy activities consistent with individual CSSP/Addendum, SMA, IAPP goals. 
  • Provides supervision and behavior management in facility and community as needed. 
  • Provides direct on-site training and instruction to participants to develop required skills to perform assigned job duties. 
  • Ensures employer and PHASE safety procedures are implemented and followed. 
  • Monitors participant’s job performance and provides for corrective action and the continued development of skills to ensure satisfactory job performance. 
  • Assists staff members in establishing criteria for satisfactory performance. 
  • Assists persons served in getting to and from vehicles, hanging up coats/wraps, putting away belongings, and other self-help areas to assist in developing self-help skills. 
  • Assists participants with personal care. Including but not limited to toileting, wiping, dressing, changing briefs, and washing participants as needed. 
  • Works with employers to ensure work performance is adequate for their needs and assures completion of job to their satisfaction. 
  • Assists with meal preparation and provides physical assistance as needed to participants during mealtimes. 
  • Serves as a PHASE liaison between employers, participants, parents and/or significant others in a manner which reflects positively on PHASE and participants. 
  • Observes, collects, and records all necessary data that relates to the participant training program and documentation when applicable. Prepares written reports as requested. 
  • Attends staff meetings, in-service sessions, workshops and classes. 
  • Attains certification and provides First-Aid and emergency CPR to persons as needed. 
  • Complies with PHASE policies and procedures as applicable. 
  • Keeps supervisor informed of job-related problems and other information relative to assigned job duties. Performs all related work as apparent or assigned. 
  • Therapeutically engages with participants at the optimal stage and ensures that the best day possible for the participant occurs. De-escalates situations calmly and effectively to ensure safety. 
  • Operates vehicles in transporting clients to and from various community destinations consistent with transportation and safety policies. May be required to use a personal vehicle for agency transportation needs. 
  • Ensures the safety of all participants when operating PHASE vehicles and adheres to the specific transportation programming requirements as directed by participant’s program plans. 
  • May be required to administer medications to participants. 
  • May be subjected to physical aggressions including but not limited to hitting, slapping, spitting, pinching, scratching, and hair pulling. 
  • May be assigned duties which require certifications. 
  • Provide support services to participants on contracted transportation vehicles. 
  • Will ensure the health of safety of the participants when riding the transportation routes. 
  • Performs other work-related tasks as apparent or assigned. 

QUALIFICATIONS FOR ENTRY: 
  • High School education or equivalent plus vo-tech/college course work in human services, special education, or a related field. 
  • Ability to develop effective relationships and work with persons with developmental disabilities. 
  • Ability to do job task analysis and implement community-based employment training programs. 
  • Ability to interact well with community employers and project a positive image of PHASE in the community. 
  • Good driving record and the possession of a current Minnesota driver’s license. Willingness and the ability to use personal automobile/vehicle for agency transportation needs. 
  • Ability to be trained and certified in First Aid and CPR. 
  • Ability to pass criminal background checks requirements, vehicle insurance qualifications, and medical examination requirements (with doctor’s certification). *Health Card.
