Outreach TBI/NHTD Service Coordinator
Job Summary: Ryrone Community Network seeks TBI/NHTD Service coordinator/ Outreach work: to work with persons who are elderly and seniors(Sign on Bonus of $2000 payable in increments)
The Service Coordinator assists the prospective participant to become a waiver participant and coordinates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, non-Medicaid Federal, State and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant's independence, productivity and integration into the community while maintaining the health and welfare of the individual. This person is responsible for individuals living at home in the community.
.Responsibilities:
- Must be willing to undergo an orientation of the DOH waiver program
- Willing to travel to various locations
- Assists the participant in the development, implementation and monitoring of all services in the Service Plans.
- Community outreach to Nursing Homes/Rehab Center, Senior Centers, Adult Day Centers
- Initiate and oversee the assessment and reassessment of the participant's level of care and on-going review of the Service Plan.
- Monthly face to face visits and documentation of visits.
- Linkage and referral to services.
- Development of individualized plan in collaboration with team
- Understanding of the NHTD and TBI waiver
- Generate referrals for home health by building relationships with physicians, long-term care, independent and assisted living facilities and other community resources.
- Generate home care patient referrals from existing and potential accounts
- Weekly meeting with owner to discuss sales progress and upcoming marketing meetings for all referral sources
Qualifications:
Service Coordinators must be a:
(A)
- Master of Social Work
- Master or Doctorate in Psychology
- Registered Physical Therapist
- Professional Registered Nurse
- Certified Special Education Teacher
- Certified Rehabilitation Counselor
- Licensed Speech-Language Pathologist
- Registered Occupational Therapist
The provider shall have, at a minimum, one (1)year of experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with Traumatic Brain Injury;
OR
(B) Individual with a Bachelor's degree and two (2) years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with Traumatic Brain Injury;
Job Type: Full-time
Pay: $65,000.00 per year
