Position: School Nurse- LPN (Essential Skills)
Reports to: Health Services Nursing Supervisor and Assistant Superintendent
Please be advised that there is 1 (one) full-time position for the following school effective immediately.
  1. Woodland Elementary School, 40 hours/week (full-time)
Position Details:
  • 181-184 day position that follows the (180) student calendar (plus 1-4 days before and/or after the school year)
  • $25.43 per hour (LPN)

Qualifications:
A. Licensed as a Registered Nurse through Indiana State Board of Nursing
B. CPR certification
C. Experience and ability to work with children
D. Good physical and emotional health
E. Proficient in use of computer, knowledgeable with basic computer software, able to operate standard office equipment, and use a telephone
Physical Requirements:
A. Ability to stand, sit, walk, bend over, reach overhead, grasp, push, pull, squat, kneel, and climb
B. Ability to perform repetitious hand and body movements
C. Ability to use hands to handle, finger, and feel
D. Ability to hear and understand speech at normal levels and on the telephone, and to speak so that others may understand at normal levels and on the telephone
E. Ability to see and read a computer screen and printed matter with or without vision aids; close vision, depth perception and ability to adjust focus for patient examination
F. Ability to perceive smells
G. Ability to lift and/or carry 30 lbs
H. Ability to occasionally lift, push, pull, carry, and/or move patients and equipment weighing up to 100 lbs.
Areas of Responsibility:
A. Implements State-Mandated Health Programs
1. First Aid/Emergency Care (IC 20-1-1-6)
a. Provides first aid/emergency care for illness or injury occurring in school or while the student is under the jurisdiction of the school
b. Acts as a consultant for such care when not in the school building
c. Informs parents and recommends follow-up care as needed
d. Provides first aid supplies to assigned buildings and for various student activities as requested
e. Maintains records
2. Communicable Disease Prevention (IC 20-8.1-7-8)
a. Helps control illnesses due to specific infectious agents or their toxic products according to public health laws
b. Screens for pediculosis and observes for other contagious diseases
c. Informs parents, explains policies, and recommends follow-up care as needed
d. Cooperates with local and state health departments
e. Maintains records
3. Immunizations (IC 20-8.1-7-9.5)
a. Initiates, annually reviews, and updates immunization records in accordance with state laws and school policies
b. Assists families in obtaining required immunizations for students
c. Maintains records
4. Vision screening (IC 20-8.1-7-16)
a. Performs annual visual acuity evaluation of students in grades kindergarten or 1, 3, 5, 8, new students, and any students suspected of having a vision problem
b. Works with local eye specialist to implement the Modified Clinical Technique vision examination
c. Refers students with possible visual defects for medical evaluation
d. Assists families in obtaining medical eye exams and glasses for students as needed
e. Maintains records
5. Hepatitis B, Universal Precautions (IC 20-8.1-7-21)
a. Conducts in-service training for faculty and staff
b. Provides information concerning blood-borne pathogens and universal precautions
c. Acts as a resource to faculty, staff, students, and parents
d. Makes referrals to appropriate medical resources
e. Maintains records
6. Reports required by the Indiana Department of Health and the Indiana Department of Education (410 IAC 3-1-4)(410 IAC 1-14)
a. Compiles statistics on chronic conditions and from immunization records and vision screening programs
b. Files reports with state departments or Health Services Nursing Supervisor by due dates
B. Performs Health Teaching and Health Counseling
1. Provides health teaching for the care and prevention of health problems
2. Provides health counseling to assist students, parents, faculty, and staff in making health-related decisions
3. Assists students, parents, faculty and staff to recognize and assume responsibility for their own health care needs
4. Makes referrals to appropriate school and/or community resources
5. Maintains records
C. Participates in the TSC Health Education Curriculum
1. Serves as a resource for the program, faculty, students, and parents
2. Assists with elementary dental health education
a. Teaches students proper tooth brushing, flossing, and safety
b. Facilitates an environment for good dental health
c. Refers students for dental care as needed
d. Assists families in obtaining dental care for students when necessary
e. Maintains records
3. Assists with human growth and development education
a. Serves as a resource for teachers, students, and parents
b. Assists teachers in grades 4, 5, 9-12 with classroom teaching
D. Supervises and/or Administers Medication and Medical Procedures
1. Oversees medication administration to ensure TSC policy and state laws are followed
2. Oversees medical procedures to ensure TSC policies and procedures are followed
3. Informs students, parents, teachers and staff about possible side effects and emergency situations that may occur from the medication and/or medical procedure
4. Maintains records
E. Participates in Kindergarten Round-Ups
1. Consults with parents to obtain health history and immunization records
2. Develops health care plans for coming year
3. Consults with other school team members
4. Maintains records
F. Maintains Record of Care Provided to Students in Health Office
1. Documents care given to students
2. Follows TSC policies and procedures as well as the school physician's recommendations and Standing Orders
3. Provides information to faculty and parents as needed for continuity of care
4. Maintains records
G. Collaborates with Community Agencies and Health Professionals
1. Assists students and parents in identifying and utilizing available and appropriate community resources
2. Confers with GLASS, County and State Health Departments and the Indiana Department of Education as needed
3. Meets regularly with other School Nurses, Health Services Coordinator, school physician, and assistant superintendent
4. Serves as preceptor for nursing students
H. Continues Professional Education
1. Assumes responsibility for continued personal learning and development for professional competency
a. Attends workshops, in-services, classes, and seminars
b. Reads current professional literature
2. Shares new information with others as appropriate
J. Assists Families and Students with Special Needs According to Public Law 94-0142, General Education Intervention, Section 504, or Students with Acute Health Problems
1. Provides health information to assist in the establishment of the student's I.E.P
2. Makes recommendations related to health and safety
3. Informs teachers as needed of those students who have chronic or temporary health conditions
4. Attends case conferences when necessary
5. Maintains records
K. Assists in the Formation of Health Policies, Goals, and Objectives for TSC
L. Carries out Other Nursing Functions as Assigned by the Administration
M. Supervises Health Assistants in Assigned Buildings
N. Uses Effective Written, Verbal, and Nonverbal Communication
O. Maintains a Professional Appearance with Grooming and Dress According to TSC Policy
P. Maintains Confidentiality of Student and Staff Health Information
