- Complete initial assessment and identification of various service needs, referral to RW support services, and determination of referral to MCM when conducting NMCM case management.
- Provide direct social work/case management services to People living with HIV/AIDS (PLWHA) and their families in accordance with their medical diagnosis and psychosocial, social and financial needs. Scope of services includes screening, assessment, crisis intervention, counseling, advocacy, information/referral, and care planning and coordination.
- Develop a comprehensive, individualized service plan when necessary.
- Respond to multiple referrals from community organizations, health department personnel, and community health providers to offer information regarding services
- Proficient in accurate documentation in clients Electronic Medical Record and Careware
- Provide assistance with the referral process by follow up of referral appointments
Position is exempt and eligible for flexible work once probationary period is complete. In office days will be required and are subject to change based on program and position needs.
- Bachelor's degree in relevant discipline or equivalent work experience required
- Proven experience as supervisor or relevant role
- 5 years of Case Management experience
- 3 years of Supervision experience or equivalent people and program management experience
- Detailed knowledge of Ryan White Case Management standards
- Ability to work independently or as part of a team
- Must be able to perform daily activities with minimal supervision
- Must have excellent written and verbal communication skills; empathy, compassion, and confidence
- Must have ability to work and communicate effectively with individuals from culturally and socioeconomically diverse backgrounds
- Master's degree in relevant discipline
