RNAC Registered Nurse Assessment Coordinator / MDS Coordinator Needed for The Hill at Whitemarsh in Lafayette Hill, PA 
 
Salary Up to 95K Depending upon Experience

What is the RNAC / MDS Coordinator role at The Hill? 
The RNAC MDS Coordinator is responsible for driving excellence in the healthcare delivery to residents of The Hill at Whitemarsh Company. The RNAC's goal is to maintain a 5 Star Rating with CMS through quality management and compliance with state and federal regulations. 
The RNAC / MDS Coordinator will have the following responsibilities: 
 

• Through monthly tracking reports, identify areas of needed improvement and provide necessary education to team members to ensure a positive outcome in Quality Measures. 
• Completion of MDS assessments, in conjunction with other RNAC staff, to maintain compliance with state and federal regulations 
• Create dashboard metrics to measure QAPI outcomes for falls, pressure ulcers, pain and other areas identified by the monthly QM reports. Oversee the QAPI program. 
• Through effective case management, ensure optimum patient outcomes, appropriate length of stay and the highest level of documentation compliance. 

What are the requirements for the RNAC / MDS Coordinator position at The Hill? 
 

• Registered Nurse ( RN ) with at least one year experience completing MDS assessments and knowledge or Quality Measures 
• Bachelor's degree preferred 
• Strong computer skills - Word, Excel, PowerPoint, Etc.
