Clinical Systems: Complete walking rounds on your unit multiple times per shift, monitoring resident conditions, ensuring nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Know your residents, ensuring immediate investigation, understanding and follow-up regarding significant changes and accidents / incidents. Ensure Care Plans and Kardex are updated to meet resident care needs. Complete daily system audits and follow-up, including and not limited to: Admissions and discharges, Infection Control: Quarantine, COVID-19, Vaccination Status, etc., Accident and Incidents, Behaviors, Labs and Imaging, Coumadin’s, Weights, Skins, Catheters, Change of Condition, Antibiotics, Bowel and Bladder, Medication and Treatment Administration, Elopement, & Equipment and Alarms. Ensure vigilance and compliance of Infection Control Protocol. Other tasks as assigned by Director and Assistant Director of Resident Care. Completion of an accurate Minimum Data Set and development of the interdisciplinary Plan of Care. Maintains current MDS status of each patient/resident, according to state and federal guidelines as required under the Omnibus Budget Reconciliation Act (OBRA), Medicare Prospective Payment Systems (PPS) and any Medicaid payment system, as applicable. Schedules patients/residents for appropriate OBRA, Medicare PPS, and or Medicaid required assessment. Captures significant changes in resident’s status. Communicates need for change of Assessment Reference Date (ARD), resident condition or look back, and reports findings to the facility Rehabilitation Coordinator. Maximizes case mix data to bolster reimbursement; maintains medical records and documentation to support the MDS and Care Area Assessment (CAA’s). Participates in weekly utilization review meetings to maximize full reimbursement. Unit Staffing: Use UKG dimensions daily to understand staff and staffing levels in your unit. Work with Staff Scheduler to ensure staffing levels meet expectations. Understand your budgeted staff levels, by unit and PPD. Ensure team members are getting their allotted breaks and lunches to reduce staff burn-out, reduce OT and stay within budgeted PPD. If staffing levels are below expectation, step in and fill gaps on your unit: Mentor and assist your team, where needed to reduce stress, ensuring the highest level of customer experience possible, when staff challenged. Mentor and guide your team by prioritizing daily activities, time management, etc. Practices within the CMS-Federal/State guidelines and maintains facility compliance and quality of care. Administrative on call rotation is required by all key operational team members. On call duties include conflict resolution, staffing needs, delegation, and resolution of customer service concerns. Attends all mandatory education and in-services (i.e., team training, safety, infection control, QAPI, UR etc.); completes mandatory health requirements. Performs staff LPN or RN duties as required or as subject to on-call (refer to the LPN or RN job description). Employee performs within the prescribed limits of the hospital’s and department’s Ethics and Compliance program and is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance Officer, or the hospital hotline.
Requirements
Completion of a state accredited school of registered nursing; must possess an Ohio Registered Nurse with a minimum of (3) years progressively responsible experience in long term care or equivalent combination of training and leadership experience. Has experience with MDS assessments and knowledge of Case Mix, Medicare, and Medicaid reimbursement process. Must possess and maintain licensure as Registered in the State of Ohio; BLS certified; may be required to obtain additional certifications as related to duties.
