We are seeking an experienced Pediatric RN Case Manager to join our healthcare team.
The Pediatric RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end of life care in the community setting. The RN Case Manager is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.
Pediatric hospice experience is required.
