Brief job description:
Administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.
- Schedule: Monday through Friday 8 am – 5 pm
- COVID Vax or Exemption Accepted: Either
- Guaranteed Hours: 40 Expected
RN Home Health experience required:
- 2+ years Home Health RN experience
- Homecare Homebase Software Experience
- Current state license as a Registered Nurse
- Proof of current CPR certification
- Current driver’s license
- Must have reliable transportation and insurance
- OASIS Experience – preferred
