Role: Community Health Worker (CHW)

Job Overview:

As a Community Health Worker at Pairtu, you will play a crucial role in promoting wellness and supporting individuals in navigating their health and social services. Utilizing our cutting-edge platform, you will engage with communities and individuals, providing personalized support and advocacy to address the Social Determinants of Health. Your work will include a mix of remote interactions and fieldwork, ensuring a comprehensive approach to health navigation.

Responsibilities and Duties:

• Utilize the Pairtu platform for care coordination, case management, and resource access among individuals and communities.
• Conduct outreach through various channels, including community events, home visits, and digital communication.
• Provide culturally appropriate health education and resources.
• Advocate for individuals and communities, focusing on housing, food security, and other critical needs.
• Deliver coaching and support for nutrition, mental health, substance use recovery, and chronic disease management.
• Foster community participation in health assessments and education initiatives.
• Embrace and practice cultural humility and awareness.

Minimum Qualifications:

• Spanish is a requirement
• High school diploma or equivalent; higher education in social sciences, public health, or related fields is a plus.
• At least one year of related experience in health services, community organizing, or advocacy.
• Access to a reliable computer and transportation.
• Proficiency in English; additional languages are highly beneficial.

Licenses:

• Valid State Driver’s License (preferred).
• Community Health Worker certificate (preferred).
