Rn Director Of Care Management

The RN Director of Care Management is responsible for the achievement of optimal health, access to services, advocacy, appropriate utilization of resources and collaboration with post-acute care providers to ensure patient's needs are met in the community. The Director of Care Management utilizes the following processes to meet the patient's individual healthcare needs: assessment, planning/intervention, implementation, Care Coordination, monitoring, evaluation of the plan of Care and communication. The Director of Care Management is responsible for the strategic planning, development, administration and operations of Care Management and Social Services for the hospital along with the Transitions of Care for patients. The RN Director of Care Management has oversight over all aspects of daily operations to enable long-term and short-term goals to be achieved.

Qualifications:

• Registered Nurse required?

• Bachelor's Degree in Nursing required?

• Director of Case Management experience in an Acute Hospital setting required
  

• CCM or ACM preferred within 1styear of hire?
  

• Master's Degree preferred?
  

Responsibilities:

• Evaluates the effectiveness of Case Management services in terms of reimbursement for inpatient care?

• Coordinates, negotiates, and procures services and resources to manage the care of complex patients, with the aim of achieving high-quality and cost-efficient patient outcomes?
  

• Seeks opportunities to reduce costs while maintaining the highest standards of Care Develops clinically based Case Management, discharge planning and Care
  Coordination to ensure the appropriate utilization of resources across the Continuum of Care?
  

• Directs all Care Management/Social Service functions within the hospital and the transition of Care?
  

• Responsible for establishing and maintaining systems and processes that optimally provide for effective and efficient delivery of patient care?
  

• Works closely with leadership in establishing and maintaining systems that optimize outcomes for all entities involved?

• Integrates and coordinates all related services of Care Management within the hospital with the associated Physician Networks?
  

• Develops integrated plans that effectively transition patients throughout the continuum?
  

• Directs the processes necessary for data collection and the management of pertinent integrated information?
  

• Participates in the review, interpretation, and decision-making regarding Resource Management?
  

• Monitors and tracks variant/avoidable bed days/visits and other utilization trends?

• Creates a collaborative team both within the ministry as well as across the continuum with all departments which impact Care Management, Social Services and Utilization Management outcomes?
  

• Develops and manages operational and capital budgets, including ensuring staffing schedules are consistent with targeted productivity?
  

• Maintains comprehensive knowledge of payor and Regulatory requirements assuring that all functions meet internal, Health Plan, and regulatory body requirements?
