The purpose of this position is to orchestrate and influence the work of others to enhance Kaweah Health's mission and vision and to achieve strategic priorities of the nursing division. Demonstrates high-level leadership accountability focused on safety and the requisite infrastructures necessary to meet expectations of the patients and families served.
QUALIFICATIONS
License /Certification
Required: CA RN license
Preferred: National certification in nursing leadership Case management certification
Education
Preferred: BSN and Masters degree in Nursing, Public Health, Business, Healthcare Administration, or related discipline
Experience
Required: Five years of management experience in healthcare leadership. Three years case management experience.
Preferred: Three years of leadership experience in patient throughput or patient flow operations.

JOB RESPONSIBILITIES
Essential
Responsible for overseeing daily operations and directing all aspects of throughput related care including Case Management, Utilization Management, Patient & Family Services, Palliative Care, Patient Throughput and Clinical Documentation Improvement. Holds direct reports accountable for meeting standards of professional practice and patient care.
Directs the delivery of patient care in the areas of responsibility in concordance with the plan for the provision of Care and service, and the plan for the provision of nursing care at Kaweah. Collaborates with Directors to coordinate the work of staff to maintain effective and efficient patient care.
Responsible for assisting in achieving core measure and value-based purchasing goals. Manages financial resources.
Participates in nursing policy formulation and decision-making. Develops, implements, evaluates and, as necessary, revises the plan for the provision of Care and service in assigned services areas. Contributes to, and coordinates services with, the development, implementation and evaluation of Kaweah's plan for the provision of Care and services.
Acts as a resource person and shows evidence of interpreting hospital policy to staff, patients and visitors.
Continuously assesses and improves department performance. Contributes to the continuous improvement of patient care and services. Contributes to staff development. Provides orientation, in-services and continuing education for staff.
Demonstrates knowledge of CMS, CDPH, quality improvement organization (QIO), California Children's Services (CCS), Title 22, and Title 9 requirements related to patient care.
Communication with key constituencies, acts as resource and liaison with medical personnel and outside agencies. Enhances professional growth and maintains awareness of current industry trends, legislation or research which could impact the department and KDH
Develops and directs a case management and social work model and method, in collaboration with the medical staff, which facilitates and supports the physicians' management of their patients through the healthcare system to assure cost effective use of resources and achieve quality outcomes
Develops and implements policies and procedures consistent with Kaweah's Mission that guide and support the Provision of Services Plan.
Determines the qualifications and competence level for department staff. Makes recommendations regarding required staffing levels.
Recommends requirements for space and other resources. Participates in selecting outside sources for needed services.
Enhances professional growth and maintains awareness of current industry trends, legislation or research which could impact the department and Kaweah Health.
Responds to emergency situations, i.e. Code Blue and disasters or any situation that interferes with smooth running of the hospital. Assumes administrative responsibilities and assists in solving problems for hospital operations at all KH campuses when appropriate.
Responsible for care coordination and Care Management activities, high risk patient identification, and development of provider hospital-based disease management programs in conjunction with the population health management teams.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
Pay Range
$64,480.00 - $950,000.00
