PURPOSE
According to the prescribed policies and procedures of the organization including all applicable federal, state and local
regulations and under the general supervision of the Regional Director of Operations has the responsibility of ensuring
quality nursing care to all clients. This position reviews the care plans for completeness, updating, and appropriateness
of goals and intervention. He/She teaches, advises, and counsels regarding the care plan and is available to staff for
problem solving, consultation regarding patient assessment and care management. Classroom teaching,
communication with staff in meetings regarding the care planning process, patient assessment, and intervention are
included in this role. The Clinical Coordinator will also assume Director of Clinical Services and Coder position as needed to ensure that there
is adequate coverage for absence/vacation requests, etc.
ESSENTIAL FUNCTIONS
• Follows up on required admission, discharge, and clinical paperwork.
• Consults with staff regarding patient care issues, such as assessments, goal setting, problem identification, and
interventions.
• Reviews and approves Care Plans and progress notes on a regular basis to evaluate progress and growth as well
as problems in the nursing care delivery process.
• Collaborates with the inter-disciplinary team to collect and document information necessary for the OASIS
completion.
• Supports nursing staff to include interviewing, new hire orientation, schedule coverage, performance
evaluations, and corrective counseling.
• Completes OASIS audits and ICD-10 coding
• Assists staff in problem-solving regarding patient care issues and identifies the need for continuing
educational offerings in the area of patient care.
• Participates in Quality Improvement activities as assigned
• Handles any family/physicians/staff concerns while on duty and investigates all incidents in his/her agency.
• Reviews visit utilization for appropriateness of care guidelines and client conditions.
• Works with managers/clinicians to address trends that affect the agency's outcome and process measures
noted during OASIS review.
• Consults with appropriate clinical staff to clarify any data integrity issues and works with clinician to make
appropriate corrections per the organizational policy.
• Participates in on-call rotation as needed.
NON-ESSENTIAL FUNCTIONS
• Maintains professional atmosphere in his/her agency.
• Attends monthly staff meetings and monthly Nursing in-servicing.

patients/clients, their families and the staff. Reports all complaints made by patients/clients and/or families to
the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property
and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state and local
regulations and laws.
• Completes all annual education requirements timely, including but not limited to Compliance, Privacy,
Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of
their duties.

JOB SPECIFICATION SHEET-Clinical Coordinator
Education: Minimum of an associate's degree.
Experience: Previous management experience in Home Health or MDS experience required. Must have training in restorative
techniques as well as in the practical application
Mental: Must have the ability to speak, read, write and understand English. Must have exceptional written,
verbal, analytical, leadership and interpersonal skills.
Licensure: Current nursing license in good standing in state in which practicing
Travel: Some travel. Weekly travel to sister branch and for as needed for team meetings
Vision: Normal: Consistent with standard workflow.
