Job Description
The Clinical Manager will administer nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the care plan, under the direction and supervision of the Nurse Administrator.
Role and Responsibilities:
· Provides services in accordance care plan
· Makes the initial evaluation, Start of Care (SOC)
· Regularly reevaluates the patient’s nursing needs
· Initiates the care plan and necessary revisions
· Initiates appropriate preventive and rehabilitative nursing procedures
· Prepares clinical and progress notes for each patient visit and summary of care conferences on their patient per company policy
· Coordinates services
· Informs company personnel and patients’ physician of changes in the condition and needs of the patient
· Educates the patient and family/significant others in meeting nursing and related needs
· Understands and adheres to established company policies and procedures
· Completes certification/recertifications orders and discharges summaries
· Determines the amount and type of nursing needed by each individual patient
· Refers to Physical Therapist, Speech Language Pathologist and Occupational therapist for those patients requiring specialized skills
· Participates in peer review and Quality Assessment and Performance Improvement as assigned
· Completes and submits OASIS assessments, reassessments, transfers, Resumption of Care (ROC), discharges and significant change in condition in accordance with company defined time frames
· Appropriately utilize ICD-10 codes
· Performs supervisory visits per state regulations and company policies
· Assures staff supervision during all operating hours, per assignment
· On call coverage, on a rotating schedule
· Assist the Nurse Administrator with on going functions of the Agency
Minimum Required Qualifications
· Must possess a valid driver’s license and proof of vehicle insurance
· Must possess your own reliable vehicle
· Registered Nurse currently licensed in the state of Nebraska.
· One (1) year of Home Health experience
· One (1) year of Oasis experience
· One (1) year of Nursing management including scheduling and staffing
Preferred Qualifications
· Bilingual proficiency preferred but not required
· Registered Nurse currently licensed in Iowa is preferred but not required
Knowledge, Skills and Abilities:
· Communicate with clients and staff in a friendly and congenial manner
· Follow verbal and written instructions, as well as document services provided
· Function independently in a home setting with minimal supervision
· Comply with client privacy, confidentiality, and HIPPA Standards
· Follow Mandated Reporter of Abuse and Neglect Laws
· Ability to exercise initiative and independent judgement

Salary: $70,000.00 - $85,000.00 per year
