JOB DESCRIPTION SUMMARY
The Clinical Manager is responsible for assisting with hospice clinical services in accordance with mission and directives set forth by the Governing Body, Executive Director, and all applicable local, state, and federal rules and regulations. Responsible for assisting with the overall direction of clinical services, in accordance with mission and directives set forth by the Governing Body, Executive Director, and all applicable local, state, and federal rules and regulations; Coordinate and oversee training, continuing education, evaluation, and retention programs as needed; Participate in the Interdisciplinary Team (IDT) – in conjunction with the IDT, develop the Plan of Care for each patient and evaluate its effectiveness; Provide proactive utilization management through efficient scheduling and management of medications/supplies/equipment; Promote standards and quality through effective and continuous communication with patients and families; Complies with all laws and regulations, including ACCREDIATION AGENCIES standards.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Assist with the oversight of patient services provided by clinical
organization personnel.Assist with the assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
Assist with timely clinical desk review of referral documentation for hospice eligibility, further documentation requests and hospice covered vs. non-covered medications.
Promotes hospice referrals in the health care community and participates in community in-services.
Plans and implements Inservice and continuing education programs to meet education and training needs of organization personnel at least quarterly.
Assist with ongoing mentoring of clinical personnel.
Assist with annual evaluations on clinicians, or more frequently if indicated.
Assist with appropriate DME and supply utilization as set by the organization.
Plans and implements Inservice and continuing education programs to meet education and training needs of organization personnel at least quarterly.
Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews.
Assists in the development of organization goals.
Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance to the Accreditation Agencies Home Care standards.
Stays informed about changes in the field of nursing and hospice care, shares information with appropriate organization personnel.
In the temporary absence (in excess of 20 consecutive working days) of the Clinical Director of Clinical Services, the Clinical Manager shall be designated in writing as responsible for fulfilling the duties of Director of Clinical Services.
In the temporary absence (in excess of 14 consecutive workdays) of a Registered Nurse Case Manager, the Clinical Manager with fulfill the role of the Registered Nurse Case Manager.
Other duties as assigned by the Director of Clinical Services.
