Full Time (80 hours bi-weekly)

M-F 8:30Am - 5PM

Home Infusion (JHIS)

The Care Team Coordinator (Outpatient Coordinator) is primarily responsible for ensuring that patients receive infusion nursing visits as clinically indicated.

Responsibilities include:

phone support and triage of patient calls

communication with referral sources

communication with home infusion clinical pharmacy team and logistics regarding delivery times to support nursing visits

sending and receiving updates for/from nurses

serving as a resource for patient care and scheduling needs from nurses conducting patient visits.

Essential Functions

Serves as a clinical and informational resource for JHIS nurses and other staff regarding the plan of care for clients at home with infusion therapies. Monitors the staffing needs for JHIS nursing activity based on the census and client acuity. Interacts with patients, co-workers, visitors, and other staff, consistent with the core values of the Hospital.

Educational / Training Requirements

RN license with Bachelor’s degree in nursing preferred.

Experience with home infusion therapies preferred
