What will I be doing?
- Participates in the review and oversight of nursing and home health aide services.
- Review patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
- Participates with Director of Home Health in training both new hire and existing qualified personnel.
- Ensure timely follow-up with patient hospitalizations, order supplies, and communicate with the interdisciplinary team.
- Perform accurate data entry in electronic medical record.
- Collaborate with outside providers and Care Coordinators to ensure smooth and reliable communication of patient’s plan of care.
- Maintain compliance with and stay abreast of changing county, state, and federal regulatory initiatives affecting home health care, including Medicare Conditions of Participation.
- Understand and comply with policy and procedures set forth by Accurate Home Care.
- Additional tasks, as assigned by manager.
Education Requirements:
- Must be licensed as a Registered Nurse (RN) in Minnesota.
Additional Requirements:
- Maintain confidentiality in accordance with applicable laws and rules.
- One-year Medicare home health visit nurse experience preferred including proficiency with OASIS.
- Ability to read, write speak fluently in English.
- Confirmed negative tuberculosis status.
- Effective communication, interpersonal and critical thinking skills.
