We are looking for a Residential Care Coordinator to join our team!
The Care Coordinator ensures the well-being and holistic care of our residents. This position is responsible for conducting home visits to patients, coordinating appointments and medical records, reviewing medications, prescriptions, and services, and facilitating seamless communication with external healthcare providers. The Care Coordinator should demonstrate professionalism when dealing with patients, staff, external stakeholders, and visitors’ needs and adhere to all company policies and standards of conduct.
Responsibilities
- Conduct comprehensive assessments of residents' physical, emotional, and social needs
- Perform home visits to assess living conditions, safety, and any additional support required
- Develop individualized care plans based on assessments and collaboration with external care providers
- Schedule and coordinate medical appointments, transportation, and medication refills
- Facilitate communication between residents and their external care teams
- Maintain resident records including appointment calendar and requesting documentation from external care team
- Complete detailed records and regular reports of care coordination activities and resident progress
- Offer support to residents and their families and care teams, advocating for residents' needs
Qualifications
- Bachelor's degree in nursing, social work, or a related field (preferred).
- Previous experience in care coordination, case management, or a similar role.
- Knowledge of healthcare regulations and HIPAA compliance.
- Strong organizational and communication skills.
- Empathy and the ability to build rapport with residents and their families.
- Proficiency in using healthcare software and electronic medical records systems.
Job Types: Full-time, Part-time
Pay: $19.00 - $25.00 per hour
Expected hours: No less than 20 per week
