Type: Three-month contract, opportunity for extension or fulltime offer
Schedule: Monday – Friday, 8am to 4:30pm EST
Pay: $20.00 to $21.05 an hour
Responsibilities of the Care Coordinator:
· The Transition of Care Coordinator (TOCC) is a critical member of the Team
· Reporting to the Health Services Director or Manager, this role assists with standardizing the post-acute care process by tracking and managing the markets census and post-acute statuses for the engaged and nonengaged patients.
· The TOCC also supports the market leadership team, clinicians, and other members of the IDT with important operational and clinical tasks, as well as proactively outreaching to local facilities to verify patient records and care statuses.
· The goal is to achieve the best clinical outcomes for our patients by coordinating services at the appropriate time, appropriate location and as proactively as possible.
· It is also to ensure continuity of care during high-risk transition periods from the acute care setting to community setting or home.
· As the Ubiquity Admit/Discharge report is solely based on the data entered into the Admit section in Ubiquity, it is imperative that the market TOCC enters all the ADT (admit, discharge, transfer) data daily and in real-time as soon as they are notified.
· Information is to be gathered from multiple sources as available to the market TOCC. The TOCC is expected to call hospitals and rehabilitation facilities as needed to track discharges/patient status in real time. Data collection and entry should be prioritized in the morning and updates throughout the day.
Qualifications for the Care Coordinator:
· High School Diploma or GED required.
· Ability to enter basic information into EMR
· Customer service experience (will be calling facilities to check on the patients discharge dates)
· Flexible and team oriented
