Resident Care Manager

PositionSummary: The Resident Care Manager will work directly with the Health Services Director and is a key member of an interdisciplinary team that will ensure quality care and delivery of services to the residents living in the community.

Salary Range $60,000.00- $65,000.00 DOE

Essential Duties and Responsibilities:

• Develop resident Service Plans and participate in care conferences.
• Input new resident data into Point Click Care and update current resident records.
• Coordinate care with outside providers and maintain appropriate documentation.
• Perform resident evaluations in a timely manner e.g., Pain, Diabetic, Self-Medication, etc.
• Respond to resident/family concerns identified and reported by Caregivers and Med Techs.
• Participate in providing continuing education to staff, as assigned.
• Maintain the Acuity Based Staffing Tool.
• Investigate, evaluate, and document incidents or accidents involving residents and ensure follow-up and update service plan accordingly.
• Work collaboratively with care staff and management team.
• Participate in projects, in-service training and committees as assigned.
• Update ADLs and communicate changes with physicians, care staff and families as needed.
• Follow-up on lab reports and doctor orders, as per policy.
• Other duties as assigned.
