Oregon Center for Children & Youth with Special Health Needs (OCCYSHN) is Oregon's Title V public health agency for children and youth with special health care needs (CYSHCN). We identify and address systems issues impacting children and youth with special health needs, ages 0-21. The duties of this position are in support of the mission, goals and objectives of OHSU, Institute on Development and Disability (IDD) and OCCYSHN as well as Maternal Child Health Bureau (MCHB) Title V public health goals and objectives.
The Implementation Specialist is a member of the Systems and Workforce (S&W) Development team at OCCYSHN. The Implementation Specialist will coordinate projects across the S&W body of work, including:
- Development and implementation of community-based autism evaluation teams or ACCESS teams
- Projects focused on working across systems to better integrate access to care and services for CYSHCN.
Key Responsibilities:
ACCESS Project Coordinator
- Coordinate all aspects of the ACCESS project and related autism systems work with active management of expectations, timelines, and deliverables through facilitation and coordination of action plan.
- Collaborate with ACCESS Program Director, Medical Director, and the OCCYSHN internal advisory committee. Lead efforts to develop and implement ACCESS work plan, including a plan for providing technical assistance and sustainability.
- Independently coordinate and facilitate ACCESS planning meetings; track ongoing agenda items and ensure work plan activities are implemented
- Independently coordinate and lead ACCESS learning sessions, including ECHOs and other professional development activities for the autism workforce; Coordinate CME
- Develop and disseminate information, processes and tools to support community partners in meeting ACCESS objectives
- Participate in ACCESS project budgeting, including tracking expenses and budgeting for program changes.
- Write and monitor scopes of work for ACCESS contracts; work with Administrative manager to initiate contracts and invoicing
- Communicate independently with ACCESS community partners
- Track ACCESS milestones and report in Title V Block Grant annual report.
Cross-systems Project Coordinator
- Provide time-limited project coordination within S&W bodies of work under direction of supervisor. Quality improvement projects may be related to cross-systems care coordination, the integration of health and education, workforce development or other time-limited projects.
- Track policy and systems change relevant to current projects.
- Collaborate with OCCYSHN S&W team members to ensure integration of activities across the team
- Health Care Transition Project Coordinator: The Health Care Transition (HCT) Project is focused on improving clinic implementation of pediatric to adult health care transition (HCT). Currently, OCCYSHN partners with the Oregon Department of Human Services (ODHS) and Randall Children's Hospital (RCH) Every Step Clinic to improve the clinic's preparation for and transfer of Every Step Clinic patients to adult primary health care. The Health Care Transition Project Coordinator will engage in project coordination, development, and evaluation activities.
- Facilitate core meetings and implementation team meetings, including arranging meeting logistics, preparing agendas with core team, coordinating tasks arising from meetings, taking and disseminating meeting notes; preparing for and facilitating Family and/or Youth Leader check-ins as needed; Communicating with Family and/or Youth Leaders; liaising with ODHS and RCH staff; Collating and disseminating project team email updates; and co-leading completion of transition awareness poster for clinics with the Family Involvement Manager.
- Propose ideas for HCT strategies utilized in the educational system to the COIIN work.
- Prepare draft report sections for annual Title V block grant submission and work with HCT Project Principal Investigator to finalize.
- Contribute to the development of the HCT intervention in the clinical setting. Review and summarize related literature, and contribute ideas to the intervention's development and modification as implementation unfolds.
- Contribute to the evaluation plan and its implementation.
Other
- Participate in ongoing professional development and self-assessment of competencies. Communicate training needs to direct supervisor.
- Cross-train for duties within S&W body of work.
- Block Grant planning and reporting
- Coordinate time-limited special projects, as assigned.
- Contribute to planning for regional meetings and OCCYSHN conferences.
- Other duties as assigned.
- Bachelor's in relevant field AND 1 year of relevant experience OR Equivalient combination of training and experience.
- At least 1 year of expereience in building and maintaining community partnerships, team building and supoorting team development.
- Project management experience.
- Ability to self-direct.
- Ability to manage multiple projects at once.
- Master's in Public Health, Healthcare, Education, Social Work, nursing or related fields
- 5+ years in project management.
- Experience in healthcare quality improvement.
- Knowledge of systems of care for children and youth with special health care needs; ability to engage in systems thinking for an identified population.
