We are currently seeking 1Registered Nurse for a rewarding and flexible fee for service Registered Nurse position within our Personal Care Agency. We believe that our clients and their families deserve home health care delivered with compassion, excellence, and reliability.
Field Supervisor RN are responsible for the initial evaluations as well as re-assessments of our home care clients. This offers you the ability to build lasting relationships with clients and their families. Through these relationships you will be able to make a positive impact on their day-to-day quality of life and make a difference!
These supervisory visits are required to observe clinical proficiencies to ensure that our PCW's are knowledgeable, competent, and trained properly.
The RN Sup is responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians' orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Develop a plan of care through physicians' orders, client input, and nursing assessment to include medical interventions and measured goals and outcomes.
Educate, properly orient and train caregivers to ensure the most optimal functioning level for each client. Train clients and their family members based on clients specific needs.
Coordinate the continuum of abundance of care through documentation and timely communication with the clients physician and other caregivers.
You will also be accountable for internal case management.
