Commonheart Home Hospice is looking for a qualified, dedicated and compassionate Patient Care Coordinator to join our team to help serve our clients.
The Patient Care Coordinator (PCC) works closely with the team to plan for and implement a care plan that proactively addresses symptoms and patient/ family goals of care. The PCC builds relationships with other health care providers throughout the service area to promote end of life care; through community education and support.

Job Qualifications:
- Maintains and actively promotes effective communication with residents and family members/personal representatives.
- Responsible for maintaining a positive image of Marquardt in the community keeping in alignment with our mission, vision and values.
- Supervise and direct care of other nursing personnel including RNs, LPNs and Hospice Aids.
- Support/triage patient care visits and clinical questions and serve as RN Case Manager, as needed.
- Teach appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and other as appropriate.
- Lead, collaborate, and participate in interdisciplinary team (IDT) meetings to establish, review and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
- In conjunction with Director, lead assigned Hospice region to achieve the organizational annual goals and objectives and assures implementation as scheduled.
- Support the hospice intake and referral process to ensure timely and appropriate response to inquiries and admissions.
- Work with providers and the hospice team to gather/review the patient specific medical information to ensure appropriate patient eligibility.
- Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested.
- Responsible for ensuring that all corporate policies and procedures are administered effectively and handled in a fair manner.
- Responsible for fostering collaborative relationships with area partners and for managing the admissions processes.
- Provide back up and support to other Hospice regions and/or PCCs as needed.
- Responsible for compliance with all local, state, and federal codes pertaining to the operations of the agency.
- Responsible for on-call services as assigned.
- Performs other similar duties, as needed.
- Must be currently licensed as a Registered Nurse in the State of Wisconsin.
- Must have a clear background check.
- Must have a valid driver’s license with a good driving record and auto insurance with Marquardt’s minimum coverage requirements
- Must have four to five years of Hospice experience.
- Must be sensitive to the need of older adults.
- Commitment to quality programs and services for all patients.
- Excellent communication and human relation skills.
- Ability to relate well to the patients, families, staff, and the community in general.
- Ability to maintain and protect the confidentiality of information.
- Ability to exercise independent judgment, make sound decisions and exhibit flexibility when needed.
- Thorough knowledge of current principles and practices related to Home Health & Hospice.
- Working knowledge of Federal and State regulation pertaining to Home Health & Hospice.
- Ability to provide leadership that fosters a team approach in meeting the mission of Commonheart.
Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
