Job Summary:
Constellation Hospice is seeking an RN On Call who will be responsible for ensuring that patient care is coordinated and managed appropriately. A common schedule will be from 5pm to 8:30am with 7 days on and 7 days off. The On-Call RN is responsible for ensuring that care and services are delivered appropriately. The RN will provide triage, visits and pronouncements during scheduled on-call time throughout our Houston service area.
Responsibilities:
- Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
- Conduct thorough assessments to identify patient needs and develop individualized care plans.
- Manage medications and treatments as prescribed by the physician.
- Provide emotional support and guidance to patients and their families.
- Educate patients and families about hospice care and end-of-life issues.
- Collaborate effectively with interdisciplinary team members to ensure coordinated care.
- Maintain accurate and up-to-date patient records.
- Participate in ongoing education and training opportunities to stay abreast of the latest advancements in hospice care.
Qualifications:
- Current and active RN license in the state of employment.
- Bachelor’s degree in nursing (BSN) preferred.
- Minimum of 1 year of experience in hospice care or end-of-life care setting.
- In-depth understanding of end-of-life care principles and practices.
- Exceptional communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
