Job Type: Permanent / Direct Hire
Setting: Home Health
Location: Fairbanks, AK
Schedule: Monday-Friday (5) 8hr days or (4) 10hr days. Share of on-call expected, including weekends, nights, and holidays. Mileage reimbursement
Pay: Based on experience , plus relocation assistance, tax credit, signing bonus, housing, living, and home schooling stipends
The The Hospice RN Case Manager is a vital member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The Hospice RN Case Manager is designated to provide coordination of care, ensure continuous assessment of each patient and family’s needs and implementation of the interdisciplinary plan of care.
REQUIREMENTS:
-Education: Associate Degree in Nursing (ADN) or; Bachelor of Science in Nursing (BSN) preferred
-Licensure: Current license to practice as a Registered Nurse (RN) in the state of Alaska
-Valid state-issued driver’s license, automobile insurance, and reliable transportation
-One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred
