Responsibilities:
- Conducted comprehensive in-home patient assessments and formulated care plans tailored to individual patient needs.
- Deliver skilled nursing care, including wound care, medication management, and various nursing procedures.
- Educate patients and their families on health management and preventive strategies.
- Collaborate with healthcare team members, such as physicians, therapists, and social workers.
- Accurately and promptly document patient care in electronic medical records.
- Stay updated on current nursing practices and trends within the Home Health care sector.
Who Will Be a Great Fit?
- Active RN license in the state of WV
- Minimum of one year of experience in Home Health care or a related healthcare field.
- Strong communication and interpersonal skills.
- Ability to work independently and manage multiple patients effectively.
- Reliable transportation, valid driver's license, and auto insurance.
- Proficient in using electronic medical records.
- Current CPR certification.
