Home Health Oasis Registered Nurse - Petersburg

Job Description: We are seeking a motivated and experienced Registered Nurse to join our team as a Home Health RN in Petersburg. In this role, you will be responsible for delivering comprehensive nursing care to patients in their homes, assessing their health needs, and developing individualized care plans. The ideal candidate will have a strong clinical background, excellent communication skills, and a genuine desire to make a positive impact on the lives of our patients.

Responsibilities:

• Perform comprehensive assessments of patients' health status and develop personalized care plans
• Provide skilled nursing care, including wound care, medication management, and patient education
• Collaborate with interdisciplinary team members to coordinate and implement patient care plans
• Monitor patients' progress and adjust care plans as needed to promote optimal outcomes
• Educate patients and their families on proper self-care techniques and disease management strategies
• Document all patient care activities accurately and in a timely manner
• Communicate effectively with patients, families, and other healthcare providers

Qualifications:

• Valid Registered Nurse (RN) license
• Experience with OASIS required
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Reliable transportation and a valid driver's license
• BLS through AHA or ARC
