HIRING MOTIVATED NURSES FOR SYRACUSE VAMC, SYRACUSE N.Y

The Home Based Primary Care (HBPC) Program Director is a clinician accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC, which includes all facility and CBOC locations. He/she facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members.

The HBPC Program Director is responsible for directing the clinical and administrative services offered by the program to ensure that the program follows local and VHA standards and policies as well as accreditation standards for home care organizations. He/she is responsible for the development and continued effective functioning of the interdisciplinary health care team. He/she recognizes and utilizes the skills, knowledge and contributions of each team member. He/she collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis. He/she is responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility. In addition, he/she is responsible for communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary. He/she is responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety. He/she is responsible for the evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained. He/she assures the program can optimize cost effectiveness (budget) and patient outcomes. He/she collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and coordinates a process for ongoing staff clinical competency with discipline specific supervisory staff He/she determines the continuing education needs of program staff and assures appropriate resources to meet those needs. He/she coordinates student education programs in HBPC. He/she is responsible for managing and tracking program resources to include but not limited to equipment inventory and leased Vehicles. He/she is responsible for tracking and managing incoming consults. He/she collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data. He/she has the major responsibility for implementation of the vision, mission, philosophy, and core values for the organization. He/she is responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and or accrediting bodies. In a leadership role, he/she participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring.

Preferred experience(s):

  • Prior work in home care settings with good understanding of clinical workflows and regulatory influences (internal as well as external)
  • Leadership roles managing diverse interdisciplinary teams
  • Experience with data driven program/workflow redesign and improvement
