JOB DESCRIPTION:
In this position, the Registered Nurse assures the coordination and provision of care for patients as assigned. The RN is responsible for coordinating patient care with physicians, family members, and other home care team members to assure quality care and positive patient outcomes.
JOB DUTIES:
- Provide quality nursing care for patients in their place of residence
- Analyze patient needs and administer appropriate care as ordered by the physician
- Establish and supervise the implementation of home health aide care plan as assigned
- Takes night/weekend call as needed
- Document all aspects of the nursing process with concise and pertinent information
- Participate in case conference activities in order to coordinate patient care services
- Understand and participate in quality management activities
POSITION QUALIFICATIONS:
· Must be a high school graduate or have a GED
· A minimum of one year full time experience preferred, in the last three years in direct patient care, institutional setting (hospital or nursing facility) or agency setting
· Knowledge of home care, infusion therapy and excellent assessment skills preferred
· Satisfactorily completed a competency evaluation program which complies with State and Federal guidelines
· Must have valid State of Texas RN license
· Must have reliable Transportation
· Must have current CPR Card
· Must have current TB or Chest X-ray
· Must have Social Security Card or Birth Certificate
· Must have authorization to work in the USA, if applicable

83.00 per visit (taxable) 12.00 mileage (non taxable) 95.00 total
RN- Re-cert or Post Hospital
63.00 per visit (taxable) 12.00 mileage (non taxable) 75.00 total
RN – Follow Up/Supervisory
43.00 per visit (taxable) 12.00 mileage (non taxable) 55.00 total
RN – Discharge
48.00 per visit (taxable) 12.00 mileage (non taxable) 60.00 total
