Responsibilities include developing a Plan of Care (POC) for patients based on their needs and feedback from their families, helping support adherence to the treatment plan, monitoring the patient’s needs, including wellness, medical, and specialist treatment, and recommending any community services required.
Requirements:
- Indiana professional license
- CPR certification
- Current Physical and TB test
- Experience with Home Health Software and SOC
- Valid driver's license and auto insurance
SOC rate: $90.00
