Job Description
Cross Country Workforce Solutions Group (CCWSG) is hiring Registered Nursing candidates to be considered for a Float Pool registry with a geriatric PACE facility located in Echo Park, Los Angeles, CA.
The Registered Nurse will help provide the facility with staffing needs for any call offs, paid time off, or understaffed teams.

Expectations for this Role • Serves as a critical member of the Interdisciplinary Team (IDT), contributing to insightful patient assessments, comprehensive care plans, delegation of responsibilities, and timely service determination requests • In partnership with a medical provider, leads personalized care for a panel of participants and is the point person for the IDT to ensuring progress on a participant’s plan of care.
- Delivers care with other team members wherever the participant is located (home, care center, virtual, ALF, SNF) in the areas of basic primary care, medication administration, wound care, oxygen therapy, and any other clinical needs in accordance with scope of practice
- Conducts nursing assessments, inclusive of physical, psychosocial and behavioral status
- Identifies emergency situations and independently initiates therapies as needed within scope of practice
- Educates participants, caregivers and staff on how to personalize and carry out care plans and respond to situations that arise in each participant’s life
- Monitors and evaluates therapeutic interventions, and recommends changes as needed to proactively improve health and wellbeing
- Provides on-call coverage when needed and scheduled Attributes of an Ideal Candidate • Passion and mission orientation for serving high-risk seniors and frail older adults
- Minimum 2 years of experience caring for adults, chronically disabled or frail, or elderly populations; Ambulatory care experience is helpful but not essential
- Functions independently in the day-to-day management of the clinical needs of participant and the operations of the PACE Center Clinic utilizing myPlace Health policies and procedures, leadership guidance and program goals in the decision-making process
- Active listening and conflict management skills
- Adapts quickly to multiple clinical settings
- Training in wound care, IVs, blood draws
Requirements
- Active, unrestricted RN license in California
- Graduate of an accredited School of nursing, BSN strongly preferred
- CPR and First Aid certification (within 60 days of employment)
- Alzheimer's certification
- Languages Spoken: English and Spanish preferred
- Must be able to lift and carry or otherwise move up to 50 pounds
- Assist Participants with mobility challenges
