The Role:
As a Hospice Registered Nurse Case Manager, you will play a pivotal role in providing holistic care to our patients and their families. Your responsibilities will include:
- Conducting initial assessments and developing personalized care plans.
- Administering medications and providing symptom management.
- Collaborating with a multidisciplinary team of healthcare professionals.
- Offering emotional support and education to patients and families.
- Ensuring that patients receive the highest quality of care during their end-of-life journey.
Qualifications:
- Current RN license in California.
- Hospice and palliative care experience preferred but not required.
- Strong communication and interpersonal skills.
- Compassionate and empathetic nature.
- Ability to work collaboratively in a team environment.
