Comfort Choice Hospice is a growing hospice care agency providing quality services to patients in the comfort of their homes. We are seeking a FRIENDLY and DEDICATED Registered NurseCase Manager Per Diem to plan, organize, and direct hospice care in the San Bernardino county and surrounding areas. Applicants should be experienced in nursing with an emphasis on community health education and experience.

Registered Nurse Case Manager Responsibilities:
- Performs initial client assessments.
- Oversees admissions, re-certifications, and discharges of our home care clients.
- Coordinates the appropriate care plan for clients.
- Works collaboratively with other healthcare disciplines.
- Administers medications and treatments as prescribed by the physicians in the physician's plan of care.

Registered Nurse Case Manager Requirements:
- Licensed as a Registered Nurse/RN
- Valid Driver's License and reliable vehicle with current insurance.
- Experience with Electronic Medical Records (EMR) software is preferred.
- Prior Nursing experience in a TCU, Hospice, or Home Healthcare setting.
- Demonstrated ability to work autonomously.
- Excellent at planning, organizing, and communicating.
Job Type: Temporary
Pay: $95.00 - $350.00 per hour
