Job Description
We are seeking a Registered Nurse Case Manager to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.
Responsibilities:
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous experience in nursing or other medical fields
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Observe and interpret patient’s symptoms and communicate them to physicians and pharmacy when necessary.
- Perform routine procedures (blood pressure measurements, lab draws, dressing changes, administering injections, etc.) and fill in patients’ charts
- Adjust and administer patient’s medication and provide treatments according to physician’s orders
- Foster, a supportive and compassionate environment to care for patients and their families
Skills
- Proven experience as a Registered Nurse
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills
- RN, BSN or another diploma from a nursing program
- Valid nursing license.
Current Driver’s License and vehicle insurance, and access to a vehicle are required. Ability and willingness to drive to different patients' homes.
