PRN- With Potential to Grow- Registered Nurse Case Manager
Overview:
The Nurse Case Manager in a DDA setting is responsible for overseeing and managing the healthcare services provided to individuals with developmental disabilities. This includes ensuring that all individuals receive appropriate medical care, coordinating with medical professionals, and managing Nursing and Medical Scheduling department staff.
Responsibilities:
- Utilize your expertise in case management to assess, plan, implement, monitor, and evaluate patient care
-On call rotation as assigned
- Collaborate with healthcare professionals to develop and implement individualized care plans
- Conduct patient assessments and provide ongoing monitoring of their condition
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Utilize your knowledge of physiology to interpret diagnostic evaluations and develop appropriate treatment plans
- Provide education and support to patients and their families regarding their healthcare needs
-Assure that nursing case delegation and management provided by nursing is within standards of COMAR 10.27.11, 10.22, and is evidence based best practice
- Utilize your skills in utilization management to ensure appropriate utilization of resources
- Collaborate with primary care physicians and other healthcare providers to ensure continuity of care
- Apply your knowledge of managed care principles to ensure cost-effective care delivery
- Utilize your expertise in case management to coordinate services across various healthcare settings
Skills:
- Valid RN license in Maryland
-Minimum of two years of experience in in a DDA or related setting preferred.
- 2 Years Working with Development Disabilities through the state of Maryland
- Strong knowledge of patient assessment techniques and vital signs monitoring
- Proficient in utilizing electronic medical records systems
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
- Strong organizational and time management skills
- Ability to prioritize tasks and manage multiple cases simultaneously
- Knowledge of primary care principles and practices
- Familiarity with managed care concepts and utilization management
