Job description

The Division of General Pediatrics and Adolescent Medicine is seeking a Clinical Nurse Case Manager (CNCM) to join the Pediatric and Adolescent HIV and AIDS team, starting immediately. The CNCM will work primarily in the Intensive Primary Care Clinic (IPC), managing Early Intervention Services (EIS) under the supervision of the EIS Program Director. In this role, the CNCM will provide Early Intervention Services to improve the health outcomes of adolescents and young adults living with, or at high risk for acquiring, HIV. The Clinical Nurse Case Manager will work closely with IPC and adolescent providers to identify and remove barriers to care, promote engagement in Early Intervention Services, and support the larger HIV care continuum. Medical Case Management emphasizes collaborative, coordinated care to ensure patients receive appropriate services and support for achieving long-term treatment goals. The Clinical Nurse Case Manager serves a vital role as a waypoint between patients and providers by facilitating linkages to care and engaging the patient in ongoing medical services. The CNCM will support patients through oversight of the HIV Testing/Counseling Program, outreach during face-to-face/telemedicine visits, over the phone, and via text, etc., to ensure a high quality of care for individuals seeking HIV services. The CNCM must be prepared to work in a fast-paced environment, prioritizing patient needs and managing challenging circumstances. The ideal candidate for this position would have strong communication skills, exceptional organizational, critical thinking, and decision-making skills, and a patient-centered approach to care. Candidates with a history of working with adolescent patients to provide Early Intervention Services, including Pre-Exposure Prophylaxis (PrEP) and gender-affirming care services, are preferred.


Specific Duties & Responsibilities
  • Provide global oversight of Early Intervention Services, including Health Education and Risk Reduction, HIV/Testing and Counseling, direct referrals to PrEP services, and ongoing medical case management for high-risk individuals.
  • Lead the development and formalization of a referral-based PrEP Program within the larger Johns Hopkins Children’s Center.
  • Facilitate the introduction of injectable PrEP as an innovative Early Intervention Resource for the adolescent population.
  • Manage the ongoing expansion of the EMERGE clinic, including direct Nurse Case management and the provision of high-quality gender-affirming care services.
  • Educate clients on HIV, HIV prevention services, early detection methods, rapid initiation of treatment, and ongoing medical adherence.
  • Conduct biopsychosocial assessments to identify health, social, and financial needs and take a patient-centered approach to ongoing engagement in care.
  • Confirm Ryan White (RW) Eligibility documentation, obtaining required proof of income, residency, and HIV status every six months.
  • Develop individualized care plans with the interdisciplinary team to support adolescents and their families with HIV medical care.
  • Identify and reduce barriers by engaging clients in supportive (youth-friendly) programs and facilitating access to specialized services, like gender-affirming care.
  • Coordinate and track medical care outcomes and assist with follow-up of quality indicators, including specialized medical referrals and utilization of services.
  • Triage, counsel, and refer patients with acute concerns to the appropriate providers and services, including direct nursing care, as applicable.
  • Collect specimens, review lab results, and coordinate with the lab when necessary – alerting providers of all abnormal findings.
  • Provide patient education around HIV, sexually transmitted infections (STI), disease transmission, and risk reduction strategies.
  • Review treatment regimens with clients and assist with prescription refills, connecting with the pharmacy as needed.
  • Provide HIV treatment adherence counseling through:
    • Adherence assessment/Care planning
    • HIV/ART Education
    • Pillbox preparation
    • Side effect/ Tolerance monitoring
    • Prescription completion and medication monitoring
    • Comorbidity prevention education
  • Provide targeted HIV prevention and support services, such as PrEP/PEP services, preconception counseling, tailored outreach, and youth-specific patient navigation.
  • Coordinate gender-affirming care services through the EMERGE clinic and support patients and providers through ongoing case management.
  • Complete medical record-keeping per IPC protocols and remain up to date with changes to record-keeping platforms like EPIC and Careware (HIV database).
  • Assist with clinic initiatives and additional administrative tasks, as necessary.
  • Provide health education and general support to patients, as required.
  • Perform other duties as assigned.

Additional Knowledge, Skills & Abilities

  • Basic understanding of EPIC, Caneware, and RW-specific documentation.
  • Knowledge of STI/HIV/TB/HCV disease, treatment, and resources.
  • Ability to provide tailored, solution-focused approaches to improve adherence.
  • Ability to work with culturally diverse patients, staff, and community populations.
  • Ability to work in a multidisciplinary, multi-organizational team setting.
  • Strong organizational skills with attention to detail and follow-through.
  • Self-starter with the ability to work with minimal supervision.
  • Ability to offer client-specific advocacy while upholding clinic policies.
  • Strong leadership potential.

Minimum Qualifications
  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing.

Preferred Qualifications
  • Bachelor's Degree in Nursing or related discipline.
  • Extensive experience in adolescent care and gender-affirming care services
  • Experience working with (PLWH) or other specialized populations.
  • Experience working specifically in Early Intervention and HIV case management.

Current American Heart Association or American Red Cross BCLS CPR certification required.

Must maintain current licensure and certification during duration of employment.

Acceptable completion of the JHH or BMC credentialing process.
