REGISTERED NURSE CASE MANAGER JOB SUMMARY:
- Responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.
- Manage all day to day needs of members and ensure health, safety, and welfare of all members within their caseload.
- Responsible for completing in person visits with members for acuity contact schedule, post hospital visits, and/or LTC request visits, health safety welfare visits, etc.
REGISTERED NURSE CASE MANAGER HIGHLIGHTS:
- Work schedule: Mon-Fri, 8a-5p
- Will require travel to members' homes and/or LTC facilities
- MUST live in one of the following counties to be considered : Montgomery, Clark, Greene
- Full benefits available after 90 days: Medical, Dental, Vision, Life, Short-term Disability
- 401k after 1 year of employment: With employer match and profit sharing
- The compensation for this position is $38 per hour, paid weekly
REGISTERED NURSE CASE MANAGER QUALIFICATIONS:
- Active RN License in the State of Ohio, with NO past or present actions/suspensions or probation
- Minimum 2-3 years care management, discharge planning and/or home health care coordination experience preferred
- Computer savvy- experience with MS Office programs
